Customer Per Diem Request ( maybe for V.1 still ? , definitely need for V2)

This would impact all LCs when they update SC # and free days for customers on an annual basis.

Currently this is our form and when we enter details (free days, SC # and exp date..etc) we can not even see what we are typing. the form is doesn't allow for full visibility. It is very useful but could use a revamp.
